Expected pay for this position is $46.05 - $61.02/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.
Overview
POSITION SUMMARY: The Manager, OnCall Community Health Services provides daily operational oversight and supports strategic objectives for community-based, Mobile clinic-based, virtual, and digitally enabled healthcare services. This role is responsible for developing, implementing, optimizing, and standardizing programs that improve access to care. Supports patient engagement, empowers team members and leadership to address community health needs, and promotes high-quality safe care delivery across diverse settings and care models. The Manager partners with ministry leadership, providers, care teams, community organizations, interdisciplinary teams, and key stakeholders to advance strategic initiatives, support growth, improve patient outcomes, and enhance patient experience.
Qualifications
REQUIRED QUALIFICATIONS:
- Education: Bachelor's degree in nursing
- Experience: 1 or more years of leadership in healthcare, community health, public health, population health, digital health, care management, or related healthcare operations.
- Licensure/ Certification: Current Illinois Registered Nurse License
- Other Skills/ Knowledge:
- Excellent interpersonal and communication skills.
- Solid computer skills, including proficiency with Microsoft software.
- Strong analytical and problem-solving skills, with the ability to be detail oriented.
- Leadership and team development skills.
- Strong communication, collaboration, and relationship-building abilities. Analytical and problem-solving skills.
- Ability to manage operations, resources, and performance outcomes. Proficiency with business technology and Microsoft Office applications.
PREFERRED QUALIFICATIONS:
- Education: Accompanied with a Bachelor of Nursing, master's degree in nursing, healthcare administration, public health, business administration, or a related field.
- Experience:
- Experience managing multidisciplinary teams across multiple locations or service models.
- Experience working with community partnerships, outreach programs, virtual care, digital health, or care navigation services.
- Experience with performance improvement methodologies, change management, and data-driven decision making.
