Expected pay for this position is $55.20 - $63.65/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.
Overview
OSF OnCall Digital Health is launching an innovative Primary Care Mobile Unit to bring high-quality, evidence‑based care directly to rural communities across Central, East, and Northcentral Illinois. This is a unique opportunity for an Advanced Practice Provider who thrives on independence, connection, and building something new for communities that need it most.
- Serve as the primary care provider for adults (with occasional pediatric needs), delivering both in‑person and virtual care through a digital‑first workflow.
- Practice evidence‑based medicine with no call, supported by a 24/7 digital wraparound care team.
- Build trusted relationships with rural and underserved populations by meeting patients where they are.
- Partner with community leaders and OSF leadership to shape and grow this new care model.
- Help create a culture of innovation, continuous improvement, and open communication.
- A self‑motivated, community‑minded clinician who is energized by autonomy and purpose.
- Comfortable being a champion for rural health and excited to help stand up a new, forward‑thinking care model.
- Skilled at connecting with patients and using technology to enhance access and continuity.
- Full‑time, Monday–Friday, day shift with some variability and occasional weekends.
- Travel within counties such as Fulton, Henry, Bureau, Winnebago, and LaSalle. Travel time is included in daily work hours. Overnight travel is not anticipated.
- Epic EMR; strong operational and nursing support.

- Clinical base hourly pay range for this position: $60-39 - $73.67hr. This is a salaried position.
- Range shown should be used as an estimate and can be impacted by many factors including but not limited to the critical need of the position, overall experience and qualifications, community need, and other considerations. Additional compensation components may be applicable such as bonuses or incentives which may or may not be based on metric achievement.

Qualifications
- Education
- Master’s degree in Advanced Practice Nursing or Physician Assistant Studies
- Experience
- 2+ years of experience as a Primary Care provider.
- Ability to deliver age‑appropriate, evidence‑based patient care.
- Licensure & Certifications
- Family Nurse Practitioner Board Certified OR Physician Assistant Board Certified
- Illinois Advanced Practice Nurse license and Illinois Registered Nurse license, OR Illinois Physician Assistant license
- Federal Controlled Substance License.
- BLS and ACLS certifications.
- Skills
- Strong interpersonal and communication abilities.
- Proficiency with computers and Microsoft applications.
- Solid analytical and problem‑solving skills with attention to detail.
- Background in rural health, ambulance/mobile services, or community health outreach.
- Experience with the Epic electronic health record.
