Expected pay for this position is $60.39 - $68.10/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.


Overview

POSITION SUMMARY: The Advanced Practice Registered Nurse (APRN) Virtual Care is a member of a collaborative team who is responsible for providing virtual care to a diverse population of patients across the Ministry through technology in their home environment. The APRN will perform a variety of functions including assessment, diagnosis, and intervention within their scope of practice. Utilizes evidenced based care and clinical decision support to guide practice. Performs ongoing communication and works to promote quality of care through collaboration with physicians, advanced practice providers, and other care team members, patients and families. The APRN demonstrates service excellence and accountability through recognizing the direct relationship between cost-effective and efficient provision of care and services including the impact of access to care.


Qualifications

REQUIRED QUALIFICATIONS: Education: Masters in ​Nursing Licenses/Certifications: ​Current state APRN license Current state Advanced Practice Provider controlled substance license Controlled substance registration certificate National Certification within 3 years of hire Experience: 3 years of clinical experience PREFERRED QUALIFICATIONS: Experience: FNP Working knowledge of Epic EMR System Other requirements/information: Excellent communication skills, including telephone etiquette and customer service Understanding of the principles of performance improvement, case management, discharge planning, utilization review, and medical records Understands the basic workings of Joint Commission, DRG's, and Medicare, Medicaid, and managed care reimbursement Demonstrated ability to work independently Proficiency with Windows based computer operations Proficient in utilizing an electronic EMR system
