POSITION SUMMARY: The Lead Advanced Practice Registered Provider of Digital Care is a member of a collaborative team who is responsible for providing digital care to a diverse population of patients across the Ministry through technology in their home environment. In addition to patient care, the Lead APN reviews and monitors clinical quality for a group of APNs in coordination with the medical director and clinical directors. The lead will also work with medical staff and leadership throughout OSF. Conducts regularly occurring meetings to facilitate information dissemination and support team engagement, participating in the design and implementation of APN practice quality/outcome indicators and facilitates consistent orientation processes and clinical practices across digital health programs. Works in a dyad model with the Manager and Clinical Director for OSF OnCall Digital Health.
Qualifications
REQUIRED QUALIFICATIONS: Education: Master’s degree in nursing Experience: 3 years of experience as an APRN in a family practice setting, urgent care, emergency medicine, or related area Licensure/ Certification: Reg Nurse License AND IL Advanced Prac Nurse Controlled substance registration certificate Current state Advanced Practice Provider controlled substance license Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem-solving skills, with the ability to be detail oriented. Excellent communication skills, including telephone etiquette and customer service. Understanding of the principles of performance improvement, case management, discharge planning, utilization review, and medical records. Understands the basic workings of Joint Commission, DRG's, and Medicare, Medicaid, and managed care reimbursement Demonstrated ability to work independently. Proficiency with Windows based computer operations. Proficient in utilizing an electronic EMR system. PREFERRED QUALIFICATIONS: Experience: Working knowledge of Epic EMR System
