Expected pay for this position is $25.10 - $29.54/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.


Overview

POSITION SUMMARY:

The Hospital SNF Coordinator plays a crucial role in facilitating communication and coordination between the hospital and skilled nursing facilities (SNFs). The position ensures that patients transitioning from the hospital to SNFs receive seamless, high-quality care by bridging gaps in communication, ensuring timely information exchange, and fostering positive relationships between hospital staff and SNF personnel. Acts as the primary point of contact between the hospital and SNF for resolving care coordination issues and addressing concerns regarding patient care. Works with care management and SNF teams to identify opportunities for improving patient care transitions and reducing readmission rates and facilitates initiatives aimed at improving the overall quality of care, including patient satisfaction, safety, and quality outcomes. Works with key stakeholders to establish goals, objectives, and metrics to define project success, as well as tracking, tending, and reporting of outcomes to ensure we are meeting key results.


Qualifications

REQUIRED QUALIFICATIONS:

Education:

Bachelor’s degree in business or healthcare related field

Experience: Two years’ experience as post-acute liaison, admissions coordination or equivalent to obtain knowledge of transitions from acute care to SNFs.

Other Skills/ Knowledge:

  • Solid computer skills, including proficiency with Microsoft software.
  • Strong analytical and problem-solving skills, with the ability to be detail oriented.
  • Working knowledge of a clinical EMR system.
  • Substantial interpersonal and communication skills to develop and maintain cooperative working relationships with personnel from other departments and community partners.

PREFERRED QUALIFICATIONS:

Experience: Three years of experience with post-acute facility admission referral management and insurance authorization.
