Pay range for this position is $33.36 - $50.34/hour. Actual pay is based on years of licensure.
POSITION SUMMARY:
The RN Case Manager - Inpatient is responsible for identifying, assessing, planning, and coordinating, implementing, monitoring, and evaluating options and services across the continuum of care for individuals in assigned case load. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services, as determined by individualized care plans. The Case Manager assures the designation of primary responsibility among team members to each aspect of the plan, avoiding duplication and fragmentation. The Case Manager assesses and intervenes to address the clinical, complex psychosocial/ behavioral health issues and needs associated with hospitalizations and proactive transition plans.

REQUIRED QUALIFICATIONS:
Education:
Associate’s degree in nursing; Bachelor's degree must be obtained within 3 years of hire.
Experience:
1 year of nursing experience
Licensure/ Certification:
State Reg Nurse License
Other Skills/ Knowledge:
Excellent interpersonal and communication skills.
Solid computer skills, including proficiency with Microsoft software.
Strong analytical and problem-solving skills, with the ability to be detail oriented.
PREFERRED QUALIFICATIONS:
Education:
Bachelor’s degree in nursing
Experience:
3 years of experience caring for patients with progressively increasing responsibility.
Chronic, complex illnesses adult and/or pediatric care management experience.
Licensure/ Certification:
Care Management Certification
