Overview

The RN Clinical Triage oversees an efficient referral and triage process for outpatient behavioral health services and assesses new patient referrals for appropriate level and linkage to behavioral health care.
Qualifications

Certifications:

  • Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
  • Basic Life Support (BLS) within 30 days - American Heart Association (AHA),

Education: College Diploma: Nursing (Required)

Other Requirements

Must have the ability to make clear, quick decisions about patients that would best serve the patient, community, and organization. Must have diagnostic and triage skills to efficiently and accurately assess referral appropriateness, including patients with psychiatric and medically complex history. Must have the ability to remain calm in crisis situations. Must have strong communication and listening skills to work effectively with and build rapport with patients, their families, coworkers, and providers. Ability to work effectively with a variety of personalities at various organizational levels. Must maintain confidential information and demonstrate ability to communicate only the facts to recipients or to decline to reveal information. Ability to project a professional, friendly, helpful demeanor. Strong Epic EMR knowledge and skills.

Responsibilities

Processes incoming referrals to all outpatient behavioral health clinic services by screening relevant information in the patient's chart and determining appropriate treatment disposition, level of behavioral health care, and appropriate service provider(s).Responsible for ensuring referred patients are appropriately scheduled in a timely manner.Provides assistance in crisis situations.Collaborates & communicates frequently and consistently with providers & other appropriate healthcare team members, ensuring seamless plan of care and access to services.Takes opportunities to clearly explain the referral process to patients and other individuals to facilitate efficient and accurate scheduling of appointments.Documents accurately and completely in electronic health record.Assesses & identify patients’ needs and make appropriate referrals based on patient’s needs and familiarity with behavioral health services and community resources.Assesses for, identifies and assists in mitigating barriers to care, making appropriate alternative referrals, and providing reciproal communication to referral sources.

Compensation and Benefits

The compensation range for this position is $33.11per hour - $53.88per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
