The Senior Health Equity Strategy Professional support the development of the organization's strategic direction and coordination of health care equity to reduce or eliminate health disparities. The Senior Health Equity Strategy Professional work assignments involve moderately complex to complex issues where the analysis of situations or data requires an in-depth evaluation of variable factors.- Identify policies and practices adversely affecting groups of people with social or economic obstacles to health.
- Ensure the delivery of services in a culturally appropriate and effective manner.
- Obtain and incorporate input from members who have disparate outcomes as directed by population health leadership.
- Evaluates members outward facing communications for understanding and cultural appropriateness.
- Partner with community-based organizations to address social determinants of health-related needs.
- Support the development of a pipeline for diverse health and social service providers.
- Partner with our population health enterprise team on integration of enterprise data with community partners for analysis.

Required Qualifications
- Must reside in Michigan or within 40 miles of the border in Indiana or Ohio.
- Bachelor’s degree.
- Two (2) or more years of experience in health equity, service coordination, ambulatory care, community public health, care management, or care coordination across multiple settings and with multiple providers.
- Experience working in a health care or social service setting.
- Experience with program planning, implementation, and evaluation.
- Experience in analyzing data and making data-driven recommendations for quality improvement.
- Proficiency in Microsoft applications including Word, Advanced Excel, and PowerPoint.
- Excellent interpersonal skills; ability to develop effective relationships with a broad array of people internally and externally, including community partners.
- Ability to take personal initiative and work independently, as well as part of a team.
Preferred Qualifications
- Bachelor’s or master’s degree in nursing, Public Health, Social Work, Health Services Research, Health Policy, Information Technology, or another relevant field.
- Previous Medicaid or Managed Care experience.
- Bilingual or multilingual in Spanish, Arabic, Chaldean Neo-Aramaic, or other languages. Must be able to speak, read, and write fluently in both English and the specified language without assistance. Refer to the 'Additional Information' section for details on language proficiency assessments.
Additional Information
- Workstyle: This is a remote position with travel.
- Typical Workdays and Hours: Monday - Friday; 8:00am - 5:00pm Eastern Standard Time (EST).
- Travel: Up to 25% travel across Michigan to meet community partners, and various care teams. Occasional travel to Humana’s Detroit office is also expected for team engagement meetings.
- Language Assessment Statement: Any Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.
WAH Internet Statement
To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:
- At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested.

40Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
