Job Summary:

Pathways to Success is a program for Medicaid enrolled youth in Illinois who have complex behavioral health needs and require intensive services and support. The program functions by the guiding principles of Systems of Care and Wraparound, delivering services that are family-driven, youth-guided, strengths and community-based, and trauma-informed practices. Under the supervision of the Pathways Clinical Manager, the Care Coordination Supervisor will lead a team of 8 to 10 Care Coordinators.

Essential Functions:

  • Hires, trains, leads, and manages 8 to 10 Pathways Care Coordinators
  • Oversees clinical and supportive services provided to Pathways clients and their families through direction, supervision, coaching, and evaluation of Care Coordinators
  • Guides staff and assists in the development of treatment plans and methodologies, ensuring the effective implementation of in-home and community treatment and programming for Pathways clients
  • Monitors and ensures the quality and compliance of all aspects of the work of the Care Coordinators
  • Completes program reports in an accurate, quality, and timely manner
  • Attends relevant team and agency meetings and actively participates and contributes to relevant committees
  • Reviews all required HFS required paperwork (e.g., progress notes, treatment plans and closing documentation)
  • Collaborates with community providers (e.g., psychiatrists, teachers, etc.) in providing treatment and support to youth and families

Qualifications:

  • Master’s degree in psychology, counseling, social work, or related field required
  • Illinois QMHP required
  • LCSW, LCPC, or LMFT preferred
  • Three years supervisory experience required
  • Five years’ experience working with youth in a clinical capacity
  • Able to maintain a flexible schedule with the ability to respond to incidents of clients in crisis
