About the position:
The Maternal and Child Health nurse provides home visiting services to families living in Peoria county. The home visits include perinatal health assessments, family case management, and coordination of medical and social services. The nurse provides maternal and newborn health assessments, education, screenings, and connections to community resources to help support the family prenatally and/or postnatally.
Job Function:
- Become fully trained in the maternal and child nurse home visiting protocols.
- Perform a health and physical assessment of the perinatal patient and/or infant.
- Apply knowledge and skills related to therapeutic communication, trauma-informed care, diverse family structures, social determinants of health, prenatal/postpartum health, infant health, infant feeding, lactation, parent-child relationships, safe sleep, intimate partner violence, perinatal mood disorders, infant crying, and substance use to identify and support family strengths and needs.
- Assist parents in making appointments with health care providers, social services agencies, and other community referrals. Serve as a liaison between families and referrals as needed.
- Utilize professional interpreters when interacting with families with limited English proficiency.
- Care for diverse populations with cultural humility.
- Respond to client needs through flexible scheduling of home visits.
- Apply effective time management, organization, and multi-tasking skills to manage a caseload.
- Maintain timely, accurate, and thorough documentation of interactions with families in compliance with program and Board of Nursing guidelines.
- Complete other duties assigned to support the Maternal and Child Nursing program such as community engagement activities and educational presentations.
Qualifications:
- Current Illinois Registered Nurse (RN) license
- Current CPR certification
- Valid IL Driver’s License
- One year of relevant maternal and child health nursing experience is preferred (ie: OB, postpartum, nursery, lactation or home visiting experience)
- Ability to work with culturally diverse populations
- Proficient with laptops, electronic health records, and Microsoft Office applications
- Hearing and speaking ability to allow for effective oral communication of information.
- Physical and emotional stamina to handle job-related issues.
- Ability to move or lift at least 15 pounds
- Ability to ascends/descends flights of stairs as needed for home visits
