About the position:

This role is for a compassionate and detail-oriented Foster Care (FC) and Supervised Independent Living Program (SILP) Nurse. This role is vital in providing comprehensive healthcare services to foster children and young adults in SILP. The nurse will coordinate psychiatric evaluations, manage medical documentation, coordinate care, and ensure medication compliance. The nurse will collaborate with case workers, therapists, and families to support the well-being and development of the clients.

Job Functions:

  • Collaborate and coordinate psychiatric evaluations with the clients, key attendees, and psychiatrist.
  • Coordinate with FC and SILP case workers for necessary documentation.
  • Collaborate with intact family case workers on a weekly or bi-weekly basis.
  • Complete lab order and review results, notify physician(s) as warranted.
  • Manage pharmacy deliveries for FC and SILP and sorting the medications for the client-specific residence.
  • Ensure psychiatric medication consents are completed and up to date for clients.
  • Collaborate with schools and school nurses.
  • Conduct timely visits to the clients in the community residing in FC or SILP homes or houses.

Job Qualifications:

  • Associate’s degree in nursing (required), bachelor’s degree in nursing (preferred)
  • Licensed Registered Nurse (RN) in the state of Illinois
  • Adult and Pediatric CPR/AED certification
  • Valid Illinois Driver’s License
  • Experience in pediatric and psychiatric care.
  • Proficiency in using healthcare software (EPIC, MyEvolv) and telehealth platforms (Zoom).
  • Strong organizational and communication skills.
  • Ability to work both independently and collaboratively with multidisciplinary teams.
  • Compassionate and dedicated to improving the well-being of foster children and young adults.
  • The position may require occasional remote evening or remote weekend work to accommodate client needs or attend meetings. This role will primarily work in an office setting but may also be required to visit other sites of the agency as needed.
