Overview
Responsibilities
Assumes responsibility for personnel‑related functions, including interviewing, scheduling, payroll reporting, and enforcement of applicable policies, rules, and regulations.
Monitors employee performance and ensures timely communication of expectations and goals related to performance evaluations.
Ensures accurate clinical, financial, and operational documentation through established audit processes and appropriate systems.
Utilizes data, dashboards, and reporting tools to support high reliability, productivity, and evaluation of outcomes.
Promptly communicates significant patient care or personnel issues to the Nurse Manager and Director.
Assists leadership with continuous quality improvement, performance improvement, and outcome‑based initiatives to ensure best practices across care management services within the Carle Health System.
Coordinates on boarding, orientation, and system setup for new employees.
Conducts monthly rounding with staff and supports dyad partner in rounding activities as needed.
Provides direct patient care as needed, comprising approximately 20% of job responsibilities, with the remaining 80% focused on supervisory and leadership duties. Maintains a collaborative dyad partnership with regional RN Care Management Supervisor. Demonstrates ongoing knowledge of Care Management best practices, CMS guidelines, and NCQA standards, and serves as the Epic Super User for the Care Management team. Collaborates with dyad partner to manage daily nursing operations in coordination with the OPCM Nurse Manager.
