Overview
Responsibilities
Assist in the collection of data from post-acute care collaborative partners. Meet with facility on routine basis to review key quality indicators against benchmark and goals. Discuss action plans to address areas of opportunity as needed.
Conduct chart reviews and root-cause analysis for post-acute patients as needed to collect data and evaluate trends for
opportunities for improvement.
Follow up on patients who are readmitted to assure concerns and issues that led to readmissions are addressed.
Assist the post-acute care manager to identify partners who have opportunities for improvement. Assist post-acute care
manager in developing an action plan and monitor for success.
Document complaints brought up by patients and collaborative partners. Address concerns appropriately.
Serve as the liaison between the hospital and the post-acute care collaborative and facilitate communication between the
inpatient care coordinators and the SNF.
Support inpatient care coordinators and partner SNFs in transitioning patients safely.
Assist in the transition of patients to post-acute programs and eventually back to their primary care physician (PCP) and
community. Encourage continuity of care transitions to home care, hospice, etc.
Determine if a follow up phone calls should be made to patients discharged from post-acute providers.
Support SNF partners in clinical education upon request, both in person and virtually.
Assist SNF in implementing industry best practices.
Prepares "pre-charting" information for the SNFist providers as necessary to support provider visits as needed.
Utilize continuous improvement to identify opportunities to improve processes and quality of care.
Maintains relationships with skilled nursing facilities (SNF) that are characterized by patient-centeredness, collaboration and mutual respect. Serves as liaison between SNF and inpatient case management to ensure both parties are meeting set expectations. Identifies opportunities within SNF that will enable them to admit and successfully manage complex patients.
