Job Description

JOB SUMMARY:
As a member of the healthcare team, contributes professional knowledge and skills in the provision and management of care to Cardio-Pulmonary Rehabilitation patients.

CERTIFICATION & LICENSURE REQUIREMENTS
Registered Professional Nurse (RN) License Current licensure in good standing as a Registered Nurse in the State of Illinois.

EXPERIENCE REQUIREMENTS
1-3 years of experience in/with or specialty training in cardiovascular rehabilitation, critical care or emergency department

ADDITIONAL REQUIREMENTS
  • Use of usual and customary equipment used to perform essential functions of the position.
  • Work may occasionally require travel to other facilities/hospitals.
  • Required to drive your own vehicle for business purposes.

ESSENTIAL FUNCTIONS:
  • Assists in developing and plan of care and exercise progression for Phase II and Phase III Cardio-Pulmonary Rehabilitation patients.* Assesses patient exercise capability at time of entering the program* Participates in and shares responsibility for patient progress with Exercise Specialist, Pulmonary Therapist and Lead RN* Collaborates with physician and other members of rehabilitation team in the development and coordination of patients plan* Demonstrates the knowledge and skills needed to provide care appropriate to the age of the patients served including an understanding of the principles of growth and development over the life span* Possess the ability to assess the patient's status and interpret relevant information in order to identify each patient's age-specific requirements and to deliver the care needed in a manner that is appropriately consistent with the standards, policies and procedures in the area, department and organization
  • Utilizes the nursing process as it relates to Cardio-Pulmonary Rehab Services.* Assessment of patient * Completes Health and History questionnaire on patient * Administers depression screening to be evaluated by Behavioral Health Department * Completes nutrition assessment per dietitian * Obtains pre/post height, weight, BMI body fat, flexibility data * Administers pre/post six-minute walking test * Administers pre/post Cardio-Pulmonary Rehab quiz * Performs laboratory testing as appropriate* Plans and sets goals * Collaborates with patient and significant other * Identifies patient goals * Identifies strategies to achieve goals* Leads and supervises progression of exercise and education of patients throughout the program * Leads patients in warm-up and cool-down exercises * Monitors patient vitals and tolerance to exercise * Participates in scheduled education sessions* Evaluates patient progress to program and documents accurate patient data * Maintains accurate, clear and concise patient records * Utilizes rating of Perceived Exertion and Perceived Symptom Scales to evaluate tolerance to exercise
  • Adheres to established hospital and departmental policies, procedures, objectives, Quality Improvement Program, safety, Environmental and Infection Control Standards.* Attends orientation which covers theory and practicum relevant to assigned area* Completes a skills checklist that outlines the specific skills required to meet the nursing care needs of the patient* Attends educational offerings such as safety and infection control as required on a regular basis* Meets charting guidelines 90% of the time determined by chart audits.
  • Enhances professional growth and development.* Participates in education sessions offered to Phase II Cardio-Pulmonary Rehab patients* Attends weekly patient staff meetings* Presents in-service training for other staff* Attends community meetings and/or Central Illinois Cardio-Pulmonary Health and Rehabilitation meetings as requested by Coordinator/Lead RN* Reviews current literature: i.e. Journal of Cardio-Pulmonary Rehabilitation, etc.
