Job Description
Carle Health is seeking an Advanced Practice Provider to join our growing Primary Care Practice in East Peoria, Illinois.
Position Details Include:
- Outpatient clinic that provides Primary Care, Walk-In, and Occupational Health services
- Clinic hours are Mon - Fri from 8 AM - 6 PM
- Join an established practice with other Advanced Practice Providers and dedicated support staff
- Average number of patients per day is 18 - 35
- Fully integrated system with clinics and hospitals on Epic EMR to improve care coordination
- Located on the Illinois River, Peoria is a vibrant, affordable, and convenient city with multiple dining and entertainment venues as well as nearby residential communities with highly ranked public-school districts
- Carle Health is a not-for-profit organization with public service loan forgiveness eligibility
Education Requirements:
- Master's degree in Nursing or Physician Assistant
Licensure/Certification Requirements:
- Current Illinois registered nurse license and advanced practice nurse/physician assistant licensure
- Will need to become DOT certified within the first 3 months from hire date
