Job Description

JOB SUMMARY:
To provide leadership and oversight of the overall patient care process, assuring high performing staff provides the highest service and quality care to patients. Oversees Behavioral Health Intake Center, Community Crisis Center programs and Emergency Response Services


EDUCATIONAL REQUIREMENTS
  • Bachelor's Degree in in Nursing (BSN) or
  • Master's Degree in in the field of social work, psychology, counseling or
  • Related field

CERTIFICATION & LICENSURE REQUIREMENTS
  • Licensed Clinical Social Worker (LCSW) upon hire or
  • Licensed Clinical Professional Counselor (LCPC) upon hire.

EXPERIENCE REQUIREMENTS
  • 2 years of experience in/with patient care, documented demonstration of strong clinical and interpersonal skills

ADDITIONAL REQUIREMENTS
  • Use of usual and customary equipment used to perform essential functions of the position.
  • Work may occasionally require travel to other UPH facilities/hospitals
  • required to drive your own vehicle for business purposes


SKILLS AND KNOWLEDGE
  • Writes, reads, comprehends and speaks fluent English.
  • Basic computer knowledge using word processing, spreadsheet, email, and web browser.

  • Functions as chief retention officer for assigned programs and assists HR in recruitment efforts.
  • Can speak to turnover rate and explain any acceptable variances.
  • Acknowledges and rewards outstanding customer service and clinical excellence.
  • Assures that the care delivery team has the tools/resources to deliver outstanding service and patient care.
  • Assures that scheduling and staffing is timely, proactive, cost-effective, and meets the needs of patients.
  • Assures resource allocation and resource management are within accepted range for YTD report.
  • Uses corrective actions according to policy for education, behavior modification, and improved performance.
  • Identifies staff education needs
  • Collaborates to determine annual competencies, unit-based education, and other continuing educational needs.
  • Assures 100% staff compliance with educational expectations.
  • Evaluates staff satisfaction survey results
  • Continuously solicits feedback from staff regarding ways to improve employee engagement.
  • Collaboratively develops/implements action plans to improve employee perception.
  • Employee evaluations are done on time 100% of the time.
  • Responsible for the quality of care and for meeting quality targets within area of responsibility.
  • Participates in the oversight, maintenance, and evaluation of the plan for outcome monitoring and measurement for the target populations of patients within his/her areas of responsibility that reflect quality, service, and cost.
  • Leads interdisciplinary teams in the areas of data supported opportunities to improve program or system outcomes
  • Participates in the development, implementation, maintenance, and systematic evaluation of systems and processes that support the organizational plan for continuous quality and performance improvement.
  • Collaborates with appropriate parties in the development, implementation, maintenance, and systematic evaluation of evidenced- based pathways and clinical guidelines that ensure the provision of consistent quality and cost-effective care delivery.
  • Works with appropriate parties to assure that the care staff provides is consistent with standards of care, national patient safety goals, core measures, and other quality initiatives undertaken by the programs or the hospital/health care system.
  • Assures deliverables are met for all grants. Grant reports are submitted to grantors on time.
  • Ensures compliance with requirements set by governmental or other regulations
