Job Description

JOB SUMMARY:
The Clinical Pharmacy Specialist participates in the provision, development and coordination of comprehensive pharmaceutical care and quality assurance programs. Provides educational services and serves as a resource to personnel in the pharmacy and other hospital departments.

EDUCATIONAL REQUIREMENTS
Master's Degree in In the field(s) of Pharmacy

CERTIFICATION & LICENSURE REQUIREMENTS
Registered Pharmacist License Current State of Illinois license as Registered Pharmacist upon hire.

EXPERIENCE REQUIREMENTS
1-3 years of experience in/with an established clinical pharmacy service

ADDITIONAL REQUIREMENTS
  • Use of usual and customary equipment used to perform essential functions of the position.

ESSENTIAL FUNCTIONS:
  • PROMOTE OPTIMAL PATIENT OUTCOMES OF DRUG THERAPY BY PROVISION OF PHARMACEUTICAL CARE.* Participates in multidisciplinary patient care rounds and other patient care activities in the assigned medical unit.* Works with medical, nursing and other hospital staff to develop practice patterns to improve the quality of outcomes of drug therapy.* Provides patient counseling services.* Participates in provision of patient care activities including antibiotic control program* Serves on committees, task forces, and other groups in appropriate areas of practice.* The incumbent will participate on all care path developments that include medications.
  • TEACH AND PROVIDE DRUG INFORMATION TO PHARMACY PERSONNEL. PHYSICIANS, PATIENTS, AND OTHER HEALTH-CARE PROFESSIONALS.* Participates in clinical experiential education of pharmacy students and residents.* Participates in continuing education programs offered by the department.* Participates in educational programs for nurses, physician, and other health care professionals.* Serves as a resource and provides patient-specific information to pharmacy, nursing, and medical staff.
  • CONTRIBUTES TO QUALITY ASSURANCE, DRUG USE EVALUATION, AND PHARMACY AND THERAPEUTIC COMMITTEE ACTIVITIES.* Participates in the development and implementation of institution-wide drug use policies.* Designs and / or assists in completion of at least one drug evaluation or quality assurance project per year.* Serves as a resource for the Pharmacy and Therapeutic Committee for drug use the assigned area of practice.* Participates in adverse drug reaction and medication error reporting.
  • CONTRIBUTES TO DEVELOPMENT OF PROGRAMS AND SERVICES TO IMPROVE THE PROVISION OF PHARMACEUTICAL CARE.* Assists in the development and implementation of programs to improve the quality of outcomes of medication use.* Works with management and other pharmacy personnel to develop needed skills for pharmacy staff to implement quality improvement programs.* Serves as a role model for staff pharmacist inpatient care activities.* Reviews pharmacy and medical literature in the assigned area of practice.* Participates in pharmacy education and other conferences.* Participate in clinical research regarding drug therapy and investigational drugs.* Represents the hospital and department through speaking engagements to physicians, nursing, pharmacy, and patient groups.
