Job Description
Carle Health is seeking a Nurse Practitioner to join our growing Hospice and Home Health team in Peoria, Illinois.
Position Details Include:
- PRN position
- The Hospice NP will collaborate with the interdisciplinary Hospice/Palliative Care team in the assessment of patients and comprehensive plan of care, and management of patients who have advanced progressive chronic illnesses. They will also collaborate with the Hospice/Palliative Care Medical Directors and provide home-based hospice services
- This position will primarily consist of conducting face-to-face, in-person home visits for patients in the Greater Peoria and Bloomington-Normal regions

Education Requirements:
- Master's or Doctorate degree in Nursing
- Candidates must have at least one year of clinical experience as an Advanced Practice Provider
Licensure/Certification Requirements:
Current Illinois registered nurse license and advanced practice nurse licensure in good standing
Must have a valid IL driver's license in good standing
