Job Description

JOB SUMMARY:
Under the general supervision of the Supervisor, Outpatient Behavioral Health, will provide behavioral health assessment and treatment planning, case management, support, education, advocacy, and consultation for individuals experiencing behavioral health concerns. Will provide crisis response and intervention as needed. Function as an effective team member and represent the agency in a professional manner.

EDUCATIONAL REQUIREMENTS
Bachelor's Degree in in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field. or Bachelor's Degree in in any other field with two years of documented clinical experience in a mental health setting under the supervision of a QMHP

CERTIFICATION & LICENSURE REQUIREMENTS

SKILLS AND KNOWLEDGE
Ability to delegate wisely, exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically and think creatively. Individual will demonstrate knowledge and skills necessary to provide appropriate care to clients. Individual will demonstrate effective written and verbal skills allowing communication with physicians, co-workers, other clinic/hospital personnel, as well as the public. Position requires good organizational skills. Position requires computer skills, ability to document with client, and ability to document in a timely manner.

ESSENTIAL FUNCTIONS:
  • Key Accountability Acts as a liaison for connecting individuals engaged in hospital-based services (e.g. admitted to psychiatric unit, utilizing Emergency Department, enrolled in PHP) to community-based services. Performs diagnostic assessments, collecting diagnostic information and determining immediate treatment plans for patients. Requests medical consultation, where indicated, in a diligent and timely manner. Coordinates and consults with team members involved in client's hospital-based care. Develops treatment goals with client and coordinates with community agencies, primary care physicians, court system, and managed-care entities. Coordinates appointments for outpatient services, including psychiatric and other recommended behavioral health services. Provides crisis assessment and intervention, completes safety plan with client, and coordinates follow up care. Coordinates with Emergency Response Service and other crisis service teams as clinically appropriate. Provides support, skills training, advocacy, assistance accessing resources, and family support as needed. Maintains records in accordance with TrilliumPlace policies, as well as state and federal oversight and accreditation agencies including Joint Commission, Medicaid, Medicaid Rehabilitation Option, Illinois Department of Public Health, and Commercial Insurances. As assigned, completes Pre-Admission Screen/Mental Health (PAS/MH) for eligible individuals. Conducts and arranges for assessments needed to complete the screening process. Consults with a licensed clinician regarding findings, if applicable. Determines the service needs for individuals based on assessments and evaluations. Educates individuals and families. Links individuals to needed services. Completes all required documentation in a complete and timely manner.
  • Key Accountability Participates in in-service and competency training. Prepares special reports as needed on activities, and work group activities. Meets clinical productivity standards. Meets compliance standards for all documentation and clinical charts.
