Job Description

JOB SUMMARY:
Under the general supervision of the Community Support Supervisor, the Psychosocial Rehabilitation Program Coordinator is an experienced clinician with a clear commitment to recovery-oriented care and strong communication and program development skills. The coordinator provides for key services such as screening potential clients for admission into the program, facilitating individual and group sessions, overseeing treatment planning and case review conferences in program.

EDUCATIONAL REQUIREMENTS
Master's Degree in in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field

CERTIFICATION & LICENSURE REQUIREMENTS
Driver's License upon hire.

EXPERIENCE REQUIREMENTS
1 or more years of experience providing service to individuals with a severe and persistent behavioral health needs.

ADDITIONAL REQUIREMENTS
  • Use of usual and customary equipment used to perform essential functions of the position.


SKILLS AND KNOWLEDGE
Ability to delegate wisely, exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically and think creatively. Individual will demonstrate knowledge and skills necessary to provide appropriate care to clients with severe and persistent mental illnesses. Individual will demonstrate effective written and verbal skills allowing communication with physicians, co-workers, other clinic/hospital personnel, as well as the public. Position requires good organizational skills.

ESSENTIAL FUNCTIONS:
  • Key Accountability * Functions as the primary QMHP in the PSR program.* Provides transportation to clients as clinically appropriate. * Creates, maintains, and revises PSR schedule as necessary* Coordinates with supervisor to create, maintain and revises manuals for program groups. * Coordinates all aspects of the Psychosocial Rehabilitation Program (PSR) including: facilitating individual and group services to include, but not limited to, monitoring of daily activities, support to manage symptoms, skills training, advocacy for client's rights and preferences, assistance accessing resources, coordination of services and care.* Provides psychoeducation and office-based behavioral health services for individuals meeting qualifications for the PSR program. * Completes assessments for program referrals to determine eligibility and complete ongoing assessments and treatment planning with clients once admitted to PSR. Requests consultation, where indicated, in a diligent and timely manner. * Provides crisis assessment and intervention services, completes safety plan with client, and follows up with client following crisis situation. Coordinates with Emergency Response Service and other crisis service teams when clinically appropriate.* Maintains records in accordance with TrilliumPlace policies, as well as state and federal oversight agencies and accreditation agencies including Joint Commission, Medicaid, Medicaid Rehabilitation Option, Illinois Department of Public Health, and Commercial Insurances.
  • Key Accountability* Participates in PSR trainings, teleconferences, and project meetings arranged with DMH along with in-service and competency training. * Prepares special reports as needed on activities, and work group activities.* Meets clinical productivity standards. * Meets compliance standards for all documentation and clinical charts.
